Advanced MD is one of the platforms used to receive patient payments. Please follow instructions associated with your billing message.

"How do I update Insurance or other information?"

Call or email Monarch Diagnostics at (949) 943-0477, or [email protected].to change your insurance information on your profile. You should also email a photo of the front and back of your new insurance card to [email protected].

"What if I receive a check from my insurance provider owed to Monarch Diagnostics?"

The subscriber would need to sign the back of the check under “Endorse Here” and include “Pay to the order of Monarch Diagnostics Inc.” You may then forward the check to the address below.

"Is student testing covered by Insurance?"

Yes, the cost of student testing is covered by the student’s Health Insurance.

*Note: If the patient in question is a minor receiving the COVID-19 Test, their Parent/Guardian’s Health Insurance will be accepted. *

"What do I need to do for this PCR Testing Program?"
