From the Monarch Diagnostics Patient XChange Portal, use the credentials you created when you registered for your test to Sign In to your existing account. You may be asked to verify your identity.

From the Patient XChange dashboard, click on the “Test Results” Icon.

Click on the test you would like to view. From there, you can download or review your results in more detail.
Retrieving Your Results

When your results are available, you will receive a text and/or email notification. Follow the link in the notification to view and download your results.

This will take you to the Monarch Diagnostics Patient XChange Portal. If you have tested with us before, use your existing credentials to Sign In. If not, select the Create Account option.

You will be prompted to create a password and enter your email. This is the email that will be used to contact you about your results and other important information.

Next, you will be asked to fill out your account details. Please fill out all the requested fields.
Account Creation & Registration

Visit our website at monarchdx.com and click the "Register for Testing" button in the top right-hand corner of our menu, or in any of the other places around our site.
5.
To register for testing with Monarch Diagnostics, please fill out all the required demographic information. Make sure to accept all the Patient Consent terms, in order to receive your results in a timely and easy manner.
You are now registered to test with us, and can visit our testing center at any time during our office hours listed below. We do accept walk-ins, however it is recommended that you schedule an appointment ahead of time. Our testing team can be reached at:
